New NJ CDL Medical Certificate Process

In accordance with Federal Motor Carrier Safety Administration regulations C.F.R. 383.73(o), New Jersey commercial driver license (CDL) holders must submit a valid (non-expired) medical certificate to the New Jersey Motor Vehicle Commission (MVC). Failure to submit a valid medical certificate or a medical variance may result in your CDL being disqualified. If your CDL is disqualified, you will not be legally permitted to operate a commercial motor vehicle (CMV). If your CDL is disqualified, you may also be required to retest prior to reinstatement of your CMV operating privileges. Federal regulations regarding medical certificates encompass interstate CDL holders subject to 49 C.F.R. 391 et seq. and intrastate CDL holders subject to state regulations.

Every CDL holder licensed in New Jersey must submit a signed self certification affidavit. This affidavit is available on this website or at any MVC Agency. Please carefully review the categories listed on the affidavit and choose the category that best fits your type of driving. If you meet the excepted requirements outlined on the affidavit, please check the appropriate box for category 2 or 4. If you checked box 2 or 4, you will only need to submit the affidavit; you need not send a medical certificate or variance. If you checked box 1 or 3, you will need to submit an affidavit and valid medical certificate.

The MVC must receive your affidavit and medical certificate, or variance to update the federal commercial driver license system. Failure to do so may affect your ability to renew your CDL, or your CDL may be disqualified, resulting in the removal of your privileges to operate a CMV. Incomplete forms or an expired Medical Certificate will not be accepted for compliance.
